Healthy Neighborhoods Program

Working with the Albany County Department of Health and the Rensselaer County Department of Health, Cornell Cooperative Extension Albany County coordinates the Healthy Neighborhoods Program. This free health and safety program is available to residents in targeted zip codes-

Albany County zip codes:
12202,12206,12208,12209,12210 in City of Albany
12047 in City of Cohoes
12189 in City of Watervliet

Rensselaer County zip codes:
12180,12182, 12144 in City of Troy, Lansingburgh, and the City of Rensselaer
12033, 12090, 12123 in Villages of Castleton, Hoosick Falls, and Nassau

Trained Healthy Neighborhoods Program educators schedule a home visit and identify environmental hazards by asking residents questions from a standardized assessment form. Residents are educated about these concerns and provided referrals and follow up resources. Free cleaning, safety, and health products are also distributed to improve the home living environment. Residents can sign-up themselves and referrals are also accepted from medical entities, schools, and organizations. The Healthy Neighborhoods Program is not visiting homes at this time, but we are accepting referrals. We will schedule appointments soon.
